Clinic Information
Welcome to St. Coleman’s clinic page! Our clinic serves as a safe and supportive space where students can come if they are feeling unwell, need first aid, or have health related questions.
The clinic is located within St Coleman’s front office and is staffed by our qualified registered nurse, Mrs. Rezende, who is present every school day from 7:15am-3:15pm. Clinic guidelines, information and health related forms can be found on this page. Parents are asked to email the nurse at nurse@stcoleman.org with any concerns or questions regarding the health of their student.
Students are not permitted to carry medication on themselves during the school day. Any prescribed or over the counter medication must be kept in the clinic and administered by the nurse. The ONLY exception to this rule is for those students who are prescribed an EpiPen. Students who must carry an EpiPen at school are required to keep the EpiPen inside a waistbelt that will be worn by the student at all times while they are on campus. An EpiPen waistbelt will be provided by our school and a charge will be added your FACTS account.
A Medication Authorization form must be completed by the parent for all medications that a student will be taking while at school. A new form is required for each new school year. Please see the link below:
- Fever of 100.4 degrees or higher
- Persistent cough/sneezing
- Vomiting and or diarrhea-2 or more episodes in the past 24 hrs
- Severe cold symptoms obviously causing the student excessive discomfort such as runny nose, sore throat, congestion.
- Pink/red swollen eyes and/or yellow drainage
If your child has a fever or has been sent home from school with a fever, the student must remain fever free for 24 hrs without the aid of fever reducing medication to return to school.
*Fever reducing medications include acetaminophen (Tylenol), ibuprofen (Motrin, Advil) or any cold/flu medications that contain those ingredients.
If your student has a chronic health condition (such as severe/life-threatening allergies, diabetes, asthma, seizure disorder, etc.), please complete the form in the link below and email to the nurse. Please contact the nurse with any additional concerns or needs for your student.
Should a student present to the clinic with an illness and the nurse determines that the student should go home, the nurse will contact the parents to pick up the student. Parents must enter through the front office to pick up their student and be dismissed from school.
If your student has been ill and absent from school for 3 consecutive days, they will need a doctor’s note. The doctor’s note must be presented to the nurse upon the students return to school.
Immunizations (form DH 680) for your child must be up to date and the form turned into the school before the beginning of each school year. If we do not have the updated form on file, your child will not be allowed to attend school until received. If your child's immunizations are due to expire during the school year, please make sure to schedule their doctor’s appointment prior to the expiration date. As a courtesy, a reminder will be sent to parents via email when immunizations are due to expire.
The sports physical (EL2 form) is required to be completed every school year for students to try out or participate in any school sport. The EL2 sports physical form must be emailed to Coach Deloach or Coach Faber once completed. Please see the link below for the EL2 forms.

Particularly with younger grades, if there is a possibility of your student needing a change of clothes or undergarment during the school day, please consider packing an extra change of uniform (Including socks and underwear) in their backpack to keep at school for a wardrobe emergency.
